LETTER OF RECOMMENDATION

One Letter of Recommendation is required. The recommendation can either be from a faculty member or administrator at the college or university the applicant is currently attending, an employer or member ofthe community. The recommendation CANNOT be from a family member or friend. The recommender MUST state your full name and their relationship to you in the letter. The letter must be signed by the recommender. Please upload your letter of recommendation below. If you are unable to upload, you can have the letter of recommendation mailed directly to UCAC Scholarship Committee, P.O. Box 2095, Indian Trail, NC 28079. No faxed or emailed letters will be accepted

OFFICIAL TRANSCRIPT

An official transcript from your institution is required in order to verify enrollment, GPA and credit hours completed. Please upload your officail transcript below. If you are unable to upload, you can have the transcript mailed directly to UCAC Scholarship Committee, P.O. Box 2095, Indian Trail, NC 28079. If an official, electronic transcript is provided, it must be emailed from the school to scholarship@unioncountydst.org.

PROOF AND LENGTH OF RESIDENCY

As proof of residence in Union County, please submit a clear copy of at least one document from below:

• Driver’s License or State Issued Identification Card (Must include the address. The issue date must be at least 90 days prior the date of the application due date. You may redact the driver's license number from the copy.)

• Official Voter Registration Card, Voter ID Card, or Authenticated Voter Certification Form (Authenticated examples include, but are not limited to, raised seal, official stamp, signature) (The voter registration date must be at least 90 days prior to the application due date.)

• Current/Active Homeowners/Condominium/Renters Insurance Policy. Must provide declaration page and include the effective date of policy. Effective date of policy MUST be at least 90 days prior to the application due date.

• Deed, Mortgage or Mortgage Statement/Lease Agreement

• Utility Bill with your name and Union County address

• Bank Statement with your name and Union County address
